Nottinghamshire Transforming Care Programme 2016

The Transforming Care Programme aims to transform care and support for people with learning disabilities and/or autism who display behaviour that challenges, including those with a mental health condition.

The programme will improve the way services are delivered by ensuring care is focused on keeping individuals healthy, well, and supported in their local community. Allowing individuals to stay close to their family and friends, only using beds in hospitals where community services cannot provide safe and suitable alternatives.

Transforming Care Consultation 2016

As the new care model suggests making changes to the service that is already provides, it was important that we asked for feedback on our plans to make sure it is right.

Therefore, between 29 February and 20 May 2016 we held a 12 week public consultation. During this time we asked for your opinions on our proposed plans, checking that they reflect what individuals need to support them to stay in their community, rather than living in hospital for a long period of time.

We had a fantastic number of responses:

  • 197 completed questionnaires

  • Face to face conversation with more than 387 people

At the moment the feedback is being analysed so we can update the care model based on the comments/issues/concerns identified by you, the users, carers, families and professionals of the service.
