What is integrated care?

Integrated care aims to ‘join up’ health and social care to meet the needs of an ageing population and transform the way that care is provided for people with long-term conditions, enabling people with complex needs to live healthier, fulfilling and independent lives.

Integrated care in Nottingham City

NHS Nottingham City Clinical Commissioning Group (CCG) and Nottingham City Council, in partnership with their local workforce, NHS (Nottingham CityCare Partnership, Nottingham University Hospitals and Nottinghamshire Healthcare) and private providers, clinicians and the voluntary sector, are seeking to transform the health and social care system.

The Integrated Care programme aims to:

  1. Empower people with long-term conditions, including the frail elderly, to feel supported to manage their own health and care needs and live independently in their own homes, for longer, with less reliance on intensive care packages.
  2. Engage and enable primary care clinicians, health and social care professionals to deliver the right care at the right time in a joined up approach, improving the citizen experience of health and social care.
  3. Develop integrated and sustainable health and social care services.

What will it do for patients/citizens?

Through integrated care, work is being carried out so that patients/citizens are:

  • Being supported for the whole of their needs, with care being co-ordinated through a neighbourhood team, rather than being seen by a multitude of separate independent health and social care workers
  • Receiving early identification and intervention of on-going health and social care needs
  • Receiving improved continuity of care
  • Being empowered to help look after themselves and remain independent as much as possible
  • Receiving appropriate support before a crisis situation occurs
  • Being given improved links with services throughout the community
  • Requiring fewer and shorter visits to hospital due to improved care and support in the community

Achievements to date

The Integrated Care programme has been running since 2013. Much progress has been made during this time. Some of the important achievements made so far are listed here.
