Find out more about Nottingham’s new diabetes and cardiac services

Nottingham City Clinical Commissioning Group (CCG) is hosting an open event on Thursday 27 March to officially launch the new Integrated Diabetes and Primary Care Cardiac Services.

Patients, carers and health care professionals can come along to the Crowne Plaza Hotel to find out more about how these community-based services will benefit patients and offer care closer to home.

The Nottingham City Integrated Diabetes Service was introduced in January, and patients with Type 1 diabetes were involved in its development. Figures show that Nottingham has a high number of people being admitted to hospital for their diabetes, often early in the evening when access to help was limited. Patients also said that they wanted clinic appointments closer to home, and to be able to see a specialist in a venue other than the hospital.

The new service now offers clinics run by experienced diabetes specialist nurses, some with consultants available, and diabetes specialist dietitians will be able to help with all aspects of diet. The clinics will be held in St Ann’s, Boots in the Victoria Shopping Centre, Clifton and Dundee House at the City Hospital.

Over the last year, the CCG has been working with Nottingham CityCare Partnership to redesign the community heart failure and cardiac rehabilitation service. The new service will be known as the Primary Care Cardiac Service and will see stable angina patients who have recently been diagnosed or whose condition has recently changed. The service will be delivered from convenient locations in the community.

“We’d like to welcome patients, carers and health care professionals to join us to discuss how these services have been developed, how they will work differently, and most importantly, the benefits they will bring to patients,” said Dawn Jameson, the CCG’s Commissioning Manager - Long Term Conditions.

The event starts at 6pm with a buffet followed by the diabetes service launch, and the cardiac launch at around 7.30pm.
