Children's Continuing Care Service

NHS Nottingham City Clinical Commissioning Group has commissioned Nottingham University Hospitals NHS Trust to implement a local Children's Continuing Care Pathway in line with the National Framework for Children and Young People's Continuing Care.

Children's Continuing Care is where a specialist assessment and a multi-agency package of care is needed to support the family caring for a child or young person with multiple, complex and enduring health care needs due to disability, accident, life limiting or sudden illness.

The service will provide care for children and young people with complex health needs that cannot be met by local universal and specialist services as they usually require specialist assessment and a multi-agency package of care.

The service will offer a Single Point of Access to:
  • Accept referrals from any professional who feels a child or young person may benefit from a continuing care package
  • Conduct a multi-agency assessment including health, education and social wellbeing with the wider multi-disciplinary team
  • Develop a specialist package of care to meet identified needs in a timely manner
  • Secure appropriate agreement to fund the package
  • Monitor and review the quality and effectiveness of care packages once established
  • Ensure a smooth transition and discharge of children and young people with complex medical needs from hospital to home
  • Supporting the end of life pathway by assisting in fast tracking services.
  • Provide information and support for professionals, parents and carers relating to children's continuing care.
