Continuing Care

NHS Continuing Healthcare is a package of care which is arranged and funded by the NHS for individuals outside hospital who have ongoing healthcare needs. You can receive continuing healthcare in any setting, including your own home or a care home.

National guidance was issued in 2007 (updated 2012) which sets out a single national framework for determining eligibility for NHS continuing healthcare and NHS-funded nursing care. You can view the National Framework here.
